There is something wrong with your narrative or the deal layout.
You ruff the second round in diamond (the DK) even though you have two diamonds.
In fact on the actual layout and declarers play the slam is cold.
Maybe declarer ruffed a small diamond after cashing a top heart in dummy, a clear mistake.
